To the recently diagnosed video

Hello! Before we can record the video, we just need to capture a little information on the form below. Don't worry, this will only take two minutes and the data will only be used so that we can ensure participants well-being & for parental permission to be on the video.

Although the video will be recoded for the website, OCD-UK will make sure that everyone is ready, set up and prepared before we start recording. When you enter the zoom room, we will not be recording the session straight away.
